Hi Robert,
Sadly, iFixit doesn't have a guide for your phone, and none of the other sites I know of has one either. However, I did locate a YouTube video that seems to have pretty good instructions for doing the screen replacement you're looking for.

The screen itself you can find on several places of the internet; I usually start out by checking Amazon, eBay and AliExpress but there are likely to be several others as well. Here's the first one I came across on a cursory search as an example.
As you work, take lots of pictures so that when you're done you can create your own guide for those people who come after you with the same problem. It's fun, easy and the community will appreciate your efforts.

Rishabh, it sounds like your digitizer (touch screen) is damaged. Unfortunately, the only fix is to replace the screen. See my previous answer for information on how to do it.
