How do I replace the front digitizer

i somehow cracked the digitizer and the screen protector i had on it the the opposite of what it was supposed to do and it protected itself.

What is the model number of the tablet (not the serial number) as shown at the bottom of the back cover?

This is to ensure that the correct replacement part can be found
