Missing settings phone and message icon

My phone was working fine then my message icon phone icon and settings icon dissapperd I have tried a hard reset a soft reset and I'm still getting nothing oh and cortana is gone to

Hi!

Try using the Windows Device Recovery tool to reflash the OS to the phone. Let me know how that goes.
