Hit the sync button on the xbox console first, it should make the green ring around the power button flash just like the controller. Then power on your controller and hit the small sync button on the top of the controller near where the battery release button is.
This happened on my Moto X and it was because lint was inside the headphone jack preventing the headphones from making a solid connection. Power down the phone and use a wooden or plastic toothpick and make sure there is nothing jammed inside the headphone jack.

Sometimes there even a simpler solution, My headphone jack was having all sorts of issues. I took a flashlight to look inside and lo and behold pocket lint had built up inside the jack. I used a wooden toothpick to clean it out and had no problems after that.
I was able to search the s/n on the battery case itself and found a replacement on Ebay. I sniped the wires off the old battery and soldered the ends to the new battery and used heat shrink tubing to cover everything up. Device charged up and is good to use.
