This is a known issue with the Charge HR. It seems heat causes the glue to fail. If your Charge HR was purchased within 1 year you should contact Fitbit customer support for a replacement.
I suggest testing this by plugging the headset into a phone or computer with a headphone jack and test the audio to ensure it is the headset itself and not the adapter. If it is the headset you may need to replace the speaker on that side of the headset.
This is a tough one. I do not know that you'll be able to salvage the Touch ID functionality of the home button with the connector being damaged. You may need to purchase iPhone 6 and 6 Plus Home Button Assembly and iPhone 6 and 6 Plus Home Button Gasket. This will no longer retain the Touch ID functionality though.
