Based on this panic log it appears the front ambient light sensor has failed. Has this phone ever been repaired, dropped, or had water damage?
The good news is if it indeed is the front ambient light sensor, it can be replaced. I believe you will lose tru tone, but your phone will function and stop rebooting every 3 to 4 minutes.
