Why doesn't my headphone jack work?
My phone plays audio through it's speakers even when my headphones are plugged in.

Have you ever taken it apart? The headphone jack is modular, so it may have come loose or disconnected entirely from the board.
Try shaking the phone gently. If you hear a clink, than that may be the problem.
