Your Hard Drive fan unit has failed!
Here's a good write up on how to read the error codes. While it's a bit dated it still works! How to invoke and interpret the Apple hardware tests
So if we dissect your error code: 4MO t/4/40000003: HDD-1293
- 4MO - Fan Motor
- T - temperature
- HDD - Hard Drive Disk

Do we need to replace the HDD or we can replace only the fan to solve this issue??
You need a new fan as the fan motors tach has failed. How about creating your own question with your system details as this answer was just generic! If we know your exact model and your errors details we can guide you to the correct part & guide.
