Why is water pouring from my steaming wand

With the steaming knob in the off position and the steaming button also off. Water is dripping from the steaming wand when the machine heats up. When I start to make an espresso water then pours out the wand.

I get about 125ml of water per espresso.

The espresso also tastes weaker and has less cream. I am using the same coffee I have always used.

I have also recently cleaned the espresso boiler plate as per the manufacturers guide and also descaled the machine.

How do I fix the issue?
