What is the sound like when you use your phone in loudspeaker or 'hands free' mode on a call? I mean the sound heard by the other person.
Most modern phones have two microphones, one at the bottom of the phone (small inlet hole on bottom edge) when used in "normal' phone mode and the other when use in 'hands free' or loudspeaker mode.
It may be that the 'top' microphone (usually because it is towards the top end of the phone) is being blocked or is perhaps faulty etc.
