Hi,
Contact the Customer Care section of your mobile phone service provider, give them your phone number and once they are sure it is really you, they will give you the PUK code, which you need to enter into your phone. You will then be asked to set a new PIN code. Once you have your PUK then keep it in a safe place away from your mobile as it is the key to unblocking your SIM.
Note that if you enter a wrong PUK 10 times in a row, the SIM will be permanently blocked, and you’ll need to purchase a new SIM card.

Hi, If you still have the packaging of the SIM card, then you can probably find your PUK code somewhere close to where the PIN code is. If you do not have the original packaging or the PUK code is not there, then you should go to your operator like the others advised you to.

Hi
If the SIM card is locked you should be able to get a new network compatible SIM card (if the phone is network locked to a particular service provider) or any other network new SIM card (if the phone is network unlocked) and insert that and the phone should work. Just make sure that the new SIM card is the correct size to fit into the SIM card slot i.e. nano, micro or standard size. Check the size of the existing SIM card that is inserted now.
