My phone is getting very slow and display gets off with mixed colors?

Hello there,

I have LG v10 T-Mobile and suddenly my phones gets too slow that even if I click on something it will take about 5 seconds to show that click on the screen and when phone is not in use and I want to unlock the screen, the screen is not awaking and become dark black or some times showed multiple colors instead of turning on.
