I have this iMac model Late 2012 21.5" configured with a fusion drive 1TB+128GB SSD, I would like to swap out the 1TB Harddisk and upgraded it to maybe 512GB SSD, I probably not touch the 128GB SSD.
Is this possible, will there be an fan sensor issues? I know this would break the fusion drive which I ok for it. Thanks in advance

No you will not have fan issues. Unlike the 27" model this system uses 2.5" laptop drives.
I've been messing around with a few of our newer 21.5" iMac's and I can tell you going with a dual SSD setup doesn't buy you much here. What I would focus on is upping the RAM. And if you go this far you might look at getting a larger blade SSD replacing your 128 module.
Besides going to a smaller SATA drive can also effect the systems performance depending on what you are doing here. Before you do anything make sure to make a full backup and go to the command line to break the Fusion drive set.
Last issue here is the newer 'Thin Series' iMac's are a bear to open and if you are not careful you can damage the LCD display. Get the correct tools and follow the IFIXIT guide to the letter as it doesn't take much here to kill the display (black bars).

Thank you so much for the fast reply, actually I just didn't want to touch the default 128GB SSD which I know it is even harder to change as it requires more steps to swap it out. My Mac has 16GB Ram which was the max at that time of purchase and so I just wanted to change the default 1TB hardest to SSD to speed things up.
Do I have to issue the command to break the fusion before or after swapping the HDD to SSD.

You need to break the Fusion drive set BEFORE you take the system apart as the system will try to built a new fusion set across the new SATA SSD which you don't want to do. While I know the blade SSD is even more work its the better direction as it has the faster I/O which makes the difference! Your system is no slow poke as is! but you can make it faster running apps focusing on the RAM (which could be pushed to 32GB if you have something that really needs it (so far we haven seen the need to go that far) & blade SSD (focus here on the blade SSD). You may need to do some clean up on the HD. Try defraging the drive. I like Drive Genius there are others out there. This app also gives you some tools to measure things which may help you understand whats happening here.
