Hello. Here are some answers that I hope will help you out.

Operating system wise look for a copy of Snow Leopard - 10.6.3 is available

Mac OS uses HFS+ formatting with a GUID partition table.

The reason you saw a flashing "?" is because there was no compatible OS installed. Windows can ONLY be installed by using BootCamp inside Mac OS.

Macs do not use the alt+ctrl+delete shortcut

Solutions:

Find a copy of Snow Leopard it was the default OS when the laptop was made.

Insert the disc and hold "option" when you power on the computer.

A boot menu prompt will come up, choose the Disc.

Once inside the installer make it through the menus.

BEFORE you install the OS go to "Utilities" and select "Disk Utility"

Inside "Disk Utility" select the Hard Drive and go to "Partition"

Inside Partition set it up to use GUID Partition table and format it to Mac OS Extended (Journaled)

Name the drive the default "Macintosh HD" or whatever you want.

Then you can proceed to install a fresh copy of Mac OS.

Good luck!

For your second Q - Here's the IFIXIT Guide to replace your HD Hard Drive Replacement.

As to what HD you'll need a drive that is 2.5" 9.5mm high that is SATA II (3.0 Gb/s). Todays newest HD's are SATA III (6.0 Gb/s) which is to fast for your older MacBook.
